Successes and Challenges of Implementing and Managing a Wellness Program
Meijer Stores, a large Midwestern retailer with approx 60,000 team members was seeking to implement a Health Risk Assessment/Wellness program that had monetary rewards/penalties attached to it. Since there is an on-going participation requirement we need to track this and administer it accordingly throughout the year. The process was complicated by the need to work with several health providers, many of which had different Health Risk Assessment processes and methods to report the results back to Meijer. With nearly 30,000 team members enrolled in a health care plan, the logistics to collect the results and administer them properly is a daunting task. We used a combination of delivered and customized Peoplesoft processes to manage this. We’ll discuss our successes, challenges, and the lessons learned during this process.

Cancellations
The policy is no refunds. Requests will only be reviewed for extreme circumstances. All cancellations must be submitted in writing to Quest via myConference Cancellation Request Form
Substitutions
Substitutions for paid registrants can be made by April 3, 2008 in writing to Quest via myConference Substitutions Request Form. Changes can also be made onsite by presenting a letter on company letterhead detailing the substitution. No penalty will be applied for substitutions.
Non-Payment Clause
When you submit your registration to us, your registration becomes binding and the registration fees are due and payable. If you cancel your registration or do not attend the conference, the registration fees remain due and payable and are nonrefundable (subject to any discounts allowed under our cancellation policy). Early-bird discounts apply only when registration fees are actually received and are not based on the date you registered. If you register early, but pay after the early-bird deadline the full registration fees apply. We reserve the right to reject any registration not in compliance with our eligibility requirements or other rules or procedures. We will refund registration fees under our cancellation policy for any rejected registrations based on the date of rejection.
If Paying By Check
Print a copy of your registration confirmation letter to send in with payment. Check payable to Quest International Users Group. Mailing address to Quest, Attention: Benefits PUG Meeting Registration. Checks must be received within 30 days of your registration date and before April 3, 2008 or your registration will be cancelled and you will have to register again at the going rate. You will receive an additional payment confirmation when check is received. If the check is returned or if we cannot process it, your registration will be cancelled. Please allow 5-7 business days for the processing of your conference payment. It is important to include your registration confirmation letter with your check to ensure your funds are properly allocated.

Hotel Reservation Details:
Contacting hotels directly will not allow you the official conference rates listed below. All reservations at the special conference rates will be booked and managed through the Denver Housing, and you cannot book your initial reservation over the phone.
